The Personal Data We Collect
Should you, through your personal decision or by request of your employer or group (“Sponsor”), complete an on-line SOI Questionnaire (“Questionnaire”), the required Data collected includes your first name, last name, email address, username and password. You, or your Sponsor, may also elect to provide other Data. Should you make a purchase of any of the products or services offered on our Site, you will be asked to provide additional Data such as credit card number, expiration date, security code and billing zip code to complete your purchase. When your Data is provided by yourself, or your Sponsor, this affirms your consent for use by iDynamx.
Other Data We Collect
When you visit our Site, our server may log your computer’s internet protocol address, browser, domain, and operating system you utilized for access. This information is logged for the purpose of assuring a satisfactory user experience, diagnosing interruptions to our servers and ensuring the security of our network. Cookies may also be utilized to assist and enhance your experience. Why We Collect Personal Data
Your Data is collected for 1) the purpose of identifying your Questionnaire Results (“Results”); 2) the purpose of internal anonymized research that, when aggregated with the responses of others that have completed a Questionnaire, will be utilized to perform statistical analysis and validation studies and for the purposes of enhancing the products and services we offer; and 3) the purpose of transacting, verifying and protecting against fraud, your purchase of any of our products or services.
Consent To Collect and Use Your Personal Data
Prior to completing a Questionnaire you will be asked to confirm your consent that your Data and responses to the questionnaire will identify your Results, behavioral traits about yourself, which will be processed by iDynamx personnel, or an approved third party designated by iDynamx, for the purposes of processing, and for research, statistical and validation studies. If you complete a Questionnaire at the request of your Sponsor, you will be asked to confirm your consent that your Results will also be shared with your Sponsor. When your Results are made available to you or your Sponsor, iDynamx is not responsible for how you or your Sponsor chooses to then utilize or disclose them.
Your Rights Regarding Your Personal Data
You have the right to view and update your Data at any time. The credentials you were assigned to complete a Questionnaire are also utilized to access your Data on our Website. You have the right to request your Data be deleted in its entirety. Should you request your Data be deleted, after verification of your identity, all your Data will be deleted. Should you at a later date desire Results, you will be required to purchase and complete a new Questionnaire. If you complete a Questionnaire through a Sponsor, you have the right to delete or limit access to your Data. If you request that your Data be deleted or have limited access, after verification of your identity with an authorized representative of your Sponsor, we will delete or limit access per your request.
How We Protect Your Personal Data
We take every step to assure that your Data is maintained in a reliable environment and is utilized only for its intended use. We use reasonable and appropriate physical, technological and administrative safeguards and practices to ensure the safe and secure collection and storage of your Data and to protect against loss, misuse and unauthorized access, disclosure, alteration and destruction. Data is stored in encrypted protocols behind firewalls. Access to your Data is granted only to those that require the Data to perform their duties or provide Services.
How We Share Your Personal Data
At no time will iDynamx share, barter, trade or sell your Data without your specific written consent, except as follows: 1) with your Sponsor that requested you to complete a Questionnaire; 2) with an authorized financial institution only for the purposes of processing your personal transaction to verify your Data and protect against fraud; 3) if required by federal, state, local or foreign law, statute or regulation; 4) if in response to a lawful request from a public or government authority or agency to comply to a subpoena or other legal process; 5) to enforce iDynamx policies and procedures, protect the rights of users complying to them, or pursue remedies and limit damages if a claim is made; and 6) if the business assets of iDynamx, in whole or in part, are sold, assigned or transferred.
